Conditions for Applying in Person

If at least one of these statements is true, you must make an appointment and appear before an agent:

  • You are applying for your first passport
  • Your previous passport was issued when you were under 16
  • Your previous passport was lost, stolen, or damaged
  • Your previous passport was issued more than 15 years ago.

If none of these statements apply to you, you may be eligible to Renew by Mail using Form DS-82 (available on the U.S. Department of State Passport Forms page).

Instructions

  1. Complete Form DS-11.
  2. Provide one of the following U.S. Citizen documents (must be an original or a certified copy):
    • Valid, undamaged U.S. Passport (may be expired)
    • U.S. Birth Certificate (must have both parents' names, date filed, and seal)
    • Certificate of Birth Abroad
    • Citizenship Certificate
  3. Present one of the following IDs. Please note: if you present an out-of-state ID, you must provide an additional ID.
    • In-state, valid driver's License
    • Valid or expired undamaged U.S. passport
    • Certificate of Naturalization or Citizenship
    • Government employee ID
    • U.S. Military ID
    • Valid foreign Passport
  4. Provide a Photo
    • Smile - Passport Photo Service available on-site for a $12 fee.

  5. Bring 2 checks (bank, personal checks, or money orders are accepted- no debit or credit)
    One check will be made payable to the Department of State and the other will be made payable to the Town of Franklin.
