Need a correction to your military record? Fill out form DD-149 (PDF) and submit to your branch of service (see form for address).
There is also a state database with discharge paperwork for many Veterans who enlisted in MA or located in MA upon discharge. Please give the state Adjutant General's Military Records office a call at 508-422-1993 if you meet that criteria.
